Communication Skills Reflection: Patient Interaction This essay will examine the interaction between myself and the mother of a child who has recently been diagnosed with diabetes mellitus (Mrs X). Before examining the interaction in specific detail I would like to consider some of the more general elements that are relevant to the topic of communication between healthcare professionals and their patients. There are many definitions of communication and Wilkinson offers one definition which describes a complex process of both sending and receiving messages in different formats (both verbal, non verbal or more commonly a mixture of both elements). This interchange typically allows for an exchange of information, feelings, needs and preferences. Typically the two protagonists in a communication exchange will encode and decode messages in a cyclic pattern. Each making an analysis and response to the preceding gambit. (Wilkinson SA et al. 1999) Bugge enlarges on this definition by putting it in a context of professional nursing and suggesting that â€Å"its purpose is generally manifold but will include the means of establishing a nurse-patient relationship, to be a tool for expressing concerns or interest in the patient’s circumstances, to elicit information relevant to the patient’s condition and to provide healthcare information.† (Bugge E et al. 2006) It is an essential part of the whole process of successful communication that both parties achieve a shared understanding of meaning. This is validated by the process of feedback interpretation which indicates if the actual meaning of the message was interpreted as it was originally intended. (Coiera E et al. 1998) In this essay I should note that communication can be considered on many different levels. In this instance we are specifically considering the mechanisms of information exchange between a healthcare professional and a client which, in the broadest sense will ultimately determine many of the parameters of treatment (and also possibly patient compliance) (Stewart M 1995). Although we are specifically considering one interaction here, this does not mean that the other elements of communication are ignored. Hogard (E et al. 2001) writes extensively on the importance of communication between healthcare professionals which can cause huge problems in terms of patient management if they are anything less than optimal. I would hope that any information that I would be able to glean from a patient could be communicated to the rest of a multidisciplinary healthcare team efficiently so that appropriate management decisions could be made. If we consider an overview of a typical communicative interchange it can generally be categorised by both type and level. In the specific context of nursing, the various levels could be considered as â€Å"Social† which is considered to be safe and non-contentious, â€Å"Structuredâ€Å", which is typically utilised for situations of teaching and patient interviews and â€Å"Therapeutic† which has the characteristic of being specifically patient focussed, purposeful and generally time limited. (DAngelica M et al. 1998) Heinmann-Knoch (et al. 2005) considers the process in greater detail. If this initial interaction is successful it can develop further characteristics such as the nurse comes to regard the patient as a unique individual and begins to understand their motivations, and the patient develops a trust in the nurse. It is within this communication context that the nurse is generally able to try to provide care and, more importantly in some instances, help patient identify, resolve, or adapt to health problems We will also briefly consider the elements of both verbal and non-verbal communication. Verbal communication requires, by definition, the conscious use of the spoken or written word. The nature, grammar and syntax and context of the words chosen are important in that they can reflect the patient’s mental age, their education, their culture and in some cases their mental state and feelings of the moment. In a clinical context inferences can be made from the way the words are delivered such as their choice, their tone or pace of delivery. The characteristics most favourable for efficient and effective communication are that the words should be â€Å"simple, brief, clear, well timed, relevant, adaptable, credible†. (Philipp R et al. 2005) Non-verbal communication by contrast, relies on the interpretation of facial expressions, hand gestures, and body language. This can be an extremely subtle means of communication and can give credence (or otherwise) to the spoken word. In the nursing context, non-verbal communication can be manipulated to the nurse’s advantage to help to elicit information that may otherwise not have been forthcoming. It has been estimated that non-verbal communication accounts for up to 85% of information transfer between communicating adults. In the professional nursing context it requires both systematic observation and careful assessment and interpretation to derive the full meaning of what the patient wishes to convey. Most importantly, the nurse should be aware of incongruity between the verbal message and the non-verbal cues. The patient who smiles while describing a terrible pain is one such example. (Musselman C et al. 1999) Specific examples If we now consider elements of communication from the transcript. Clearly there is no evidence of non-verbal communication on the transcript but I was very conscious of the mother’s initial reticence in her opening exchanges with me. She initially sat in a â€Å"closed† position and largely avoided eye contact. As the conversation unfolded she clearly became more relaxed and trusting. She adopted a more â€Å"open† and relaxed posture and started to express herself with appropriate hand gestures. (Hulsman R L et al. 1999) I particularly recall the jabbing gestures she made to emphasise a point relating to giving injections. I have to observe that the environment that was used for the interview was very contrived and I believe that this may have had an influence on both myself and the client. I think that, in a real situation I would be able to allow the conversation to be far more fluid and relaxed. I can analyse some of the techniques used to elicit or reinforce information During the interview I purposely made a point of asking open questions to try to draw out the client’s response So it sounds as if, it has obviously upset you..? It sounds as if you were almost blamed yourself for it as well..? Generally its quite a healthy family as well..? >From the terminology you’re using there is sounds as if you know what you’re talking about, you sound quite confident..? Mrs X. was clearly at ease after a while and even when closed questions were asked she would answer them Yes or No and then go onto both expand what had been said and volunteer other information. I had varying degrees of success in eliciting the information that I was after. Trying to establish whether it would be difficult to get the patient to comply with his diet I touched on the subject of diets and Mrs X clearly has a major psychological difficulty in coming to terms with her own diet. I allowed her to express her views about her obesity before trying to bring the conversation back on track. After Mrs X’s outburst about her â€Å"serious morbid obesity† I made three attempts to both empathise and sympathise with her feelings in order to gain her trust by asking supportive and non-contentious questions before returning to the point relating to injections with the question â€Å"So how did you feel with the injections, because obviously for me that was quite a scary experience, seeing someone so young giving an injection to themselves..?† Phrasing the question in this way appears to show considerable empathy for Mrs X’s situation and allows her then to offer her opinion. (Richards T 1999) There were several instances where I needed to summarise what was said in order to be sure that I had understood the thrust of Mrs X’s comments Easy to understand, em, there was no panic mongering in them and things like that. If I’d have looked on the websites, I might have found some information I didn’t want to see at this stage. So I was advised not to look and I didn’t. So would you be a bit. . So if you read something that was sort of like false information or mis-information that scared you a little bit, is that what you’re trying to say? It probably would have scared me because it’s my child, but I wouldn’t have been into histrionics about it, I would have been probably saying well that†¦Ã¢â‚¬ ¦. problems. Paraphrasing was a useful technique to ensure that I had understood what was being said. Yes, and you know, instead of buying biscuits and things for the biscuit tin in the house, I’ll be buying fruit, huge varieties of fruit, and that’ll be their options now. (Stewart M . 1995) Especially being teenagers, its all.. They eat chocolate, they eat crisps and all things that are bad for them and if they’re restricted in what they can eat it makes them want it more in a way. I know if I couldn’t have chocolate or sweets.. On reflection I believe that I made a reasonable attempt in the circumstances to get as much information from Mrs X as I could. By the end of the interview Mrs X was getting tired and losing concentration so I believe that it was correct to terminate the interview at this time. (Wilkinson S et al. 1999) I am aware that I frequently asked incomplete questions or stopped in the middle of a sentence. This may be an indication that I was not in control of the situation This whole exercise has been a useful analytical and learning experience for me. I think we’ve been inundated with information, I think that it was good that one particular member of staff dealt with us mainly, and they listened very closely to what we had to say. They’re old enough to make a conscious decision, â€Å"okay Mum’s given us that, but I fancy this so I’m going to go and buy it.† They’ve got their own resources so they can go and do it themselves. And they’re quite happy about that are they? Yes, and they’re happy, apart from one, about the healthier way of cooking things if you like and em, wasn’t a terribly bad diet in the first place. Have they all agreed to sort of, give their responses to diabetes or are they sort of laid back about it? They seem quite laid back.. They are very laid back. They’ve even, I mean it sounds absolutely terrible but they’ve even been cracking jokes about it. Three of my children have got asthma and I mean he was joking â€Å"well mine’s worse than yours† and .. So they’re all quite light hearted about it and taking it in their stride. Yes. Matthew, my eldest boy, was, because he’s a Nurse I think, he was absolutely devastated but he is better about it now. But in the first instance he was absolutely gutted. I mean xxxx he was gutted obviously.. Well this is it, he seems quite calm about it from what Ive seen and a very relaxed family in general. Yes. I mean if we were any more laid back we’d need †¦Ã¢â‚¬ ¦Ã¢â‚¬ ¦.. on a night to keep us going because we don’t really let things bother us. So it sounds as if, it has obviously upset you..? I did wonder, you know, initially I wondered if it was because they’ve always been allowed to have sweets. Ive never made sweets into a reward or a treat, you know, they’ve always been an everyday matter of fact, because I didn’t want them to grow with my problems, obesity problems, because they weren’t allowed sweets. I didn’t ever want them to ever think sweets were something really special. Because they’re not, they’re just another thing that, they’re a foodstuff, and they’re a bad source of.. Especially being teenagers, its all.. They eat chocolate, they eat crisps and all things that are bad for them and if they’re restricted in what they can eat it makes them want it more in a way. I know if I couldn’t have chocolate or sweets.. Well that’s just my life experience you know, being denied things and then leaving home at 16 and eating all of that stuff and becoming at one point seriously morbidly obese I don’t want that for my children. Obviously they might have a predisposition to do that. I don’t want that to happen so, let’s not make sweets and biscuits em, a reward or a treat so they never have been. So then I wondered if possibly they’d had too much. None of them are overweight, none of them are fat, none of them have got bad teeth and cavities and things. None of them has ever had a filling. Generally its quite a healthy family as well.. He hasn’t seen a doctor for 7 years. So it must be a complete shock for you that this has suddenly happened.. and changed everything that has been happening in your home. So how has the rest of the family been coping, have they been..? Very supportive, except for one.. Ah, except for one, is that the elder one or the younger one? He’s 18 just turned, and he’s got an appalling diet, all I can do is advise him. I can put his meals in front of him and if he chooses not to eat them and go and buy rubbish then, all I can do is advise him. So how did you feel with the injections, because obviously for me that was quite a scary experience, seeing someone so young giving an injection to themselves.. I knew he could do it. Of all of this, that’s the bit that doesn’t faze him. Does it faze you at all? It doesn’t faze me. Em, when I was asked to give him an injection here, and staff knew I could do it †¦Ã¢â‚¬ ¦said you might not be able to do it because he’s your son.. But its not like that is it? Nobody’s squeamish, nobody’s terribly fazed by it, I think they’re all pretty pleased because it’s not them that’s got diabetes. Even if he’s in complete control of his diabetes will you still come in and visit just to make sure everything’s alright? Yes, as long as he wants me to come with him, then I will come with him. I mean, when he turns 18 if he wants me to come to the diabetes clinic with him then I’ll come with him. Is he independent anyway? Very. Fiercely independent. Still like, on the phone its still like Mum †¦Ã¢â‚¬ ¦Ã¢â‚¬ ¦Ã¢â‚¬ ¦ So when you go away today do you think you’ll feel as if you’re going to have everything that you need for the next, lets say two weeks, or week, to be happy with.. Well asthmatics don’t get their inhalers free, they have to pay.. When you think about it you’ve got your insulin, your sticks you†¦. It’s the keytones as well, they haven’t been completely explained to us. They’re in the leaving pack anyway which†¦ I mean I understand what the keytones are, but†¦ What are they? Because I don’t fully understand, I know they’re a by-product .. Keytones are a waste product that your kidneys filter out of the blood and pass out in the urine. Obviously they’re in your blood for your kidneys to pull them out, filter them out, but if his blood sugars are high and remain high you should test his urine for keytones. There are sticks to test his urine. If there are a lot of keytones in his urine then he should probably do a test on his blood which is just another stick that goes in Have you got them? No. Then you would phone the ward. But we know we can phone the ward anytime. So you feel reassured at having that point of contact that you can phone up anytime†¦? Absolutely, yes. Mm, very close And that was only on day two or three, day three maybe He’s had a couple of lows on his blood pressure as well which I think he’s realised because he’s felt the effect that it’s had on him He knew yesterday afternoon that he was low because he said look at me and he showed me and there was just ever such a slight tremor in his hands. Did that concern you or reassure you? It reassured me because he knew and I said, he said I need to test my blood and I said why and he said because I think its low and he said look and he showed me. For him it was low, it’s the lowest it’s been. Children are supposed to be raised by both parents, and we all accept that father and mother have vital roles in upbringing their children into potential and useful people to contribute to social development. However, it is quite challenging if we want to compare whether mother or father is more important. What if mother is more important, is she has to assume more responsibility, or what if father is, ought he to assume more? On my point of view, in no account should parents assume unequal responsibility due to children development sake, the difficulty of raising children, and family happiness. Whether or not children are nurtured with equal responsibility from mother and father has effect on childrens development throughout their lives. With both cares, children get full experience of live from both mother and father because what father can give to children is different from that of the mother, but it doesnt mean one is less important than the other. Mother can teach children how to cook, how to sew, or how to deal with household chore while father can teach them how to fish, to be strong, or to protect oneself from any danger. And some circumstances children need advice from father and other circumstances, only mother is more suitable to give advice. On top of that it is also known that with both parents care, children benefit in their braveness. Children will feel confident around the house, at school, or elsewhere because they think they are equal to other, and they have both parents supporting them. Secondly, raising children is not easy task. When they were babies, from milk, food, cloth, cleaning, learning to walk, or sleep is all stressful. They are twenty four hours needed to be taken care of. What if the baby gets sick, not only money that you have to pay but also the nervousness you encounter. As they grow up and go to school, more thing parents need to concern whether they study well, make friend with gangster, play school truant, or eat something unhealthy. Furthermore, every children is different, using old way to teach children will not work to your own children. You need more time to observe them and choose the best way. Last but not least, that both parent share equal responsibility increases family happiness. The more time you are with each other, the more understanding you have for each other. Everyone in the family feel close and warm. It brings peaceful environment to children as well the whole family. Nothing is worse than parent care only working and dont have time for family. My counterpart might say that parents cant share equal responsibility because father is busier at work to own for the living. He doesnt have time to take care of their children. Mother needs to be responsible for it. This argument has some merit on the surface. Remember marriage is not all our sharing task to do, but love. Work at house, taking care of the baby and household, is much more stressful then the job outside. In addition, everyone is busy; it is just the matter of how you manage your time, and what if mother unfortunately passes away, who will take care of our children? How could you support to take care of them when you and they are almost stranger? If father dont have time for his children, it will reduce the family happiness. To sum up, because of the interest of childrens development, the stress of raising children, and the family satisfaction, sharing equal responsibility is a must. Every parent should take this into consideration because children are weak and innocent they need both cares and support to help them accept new thing, learn the social value, and meet with the social expectation.

Stirpiculture in the Oneida Community   Ã‚  Ã‚  Ã‚  Ã‚  John Humphrey Noyes, a native of Brattleboro, Vermont, rebelled from religion from a young age and after a near death experience became devoted to the goal of being introduced to the ministry. The most influential reasoning to Noyes’ theory was that of Perfectionism, in which believers reached perfection at conversion. Following extensive failure, Noyes finally acquired a following in 1844 in which the thirty-seven members lived communally. Two years later, the prominent ideals began to originate such as â€Å"Complex Marriages† and â€Å"Male Continence.† The Oneida Community’s doctrines had many components, but the basis of the community was centered on the idea of complex marriages. The practice of complex marriages provides the source for many controversial ideas they enacted in addition to what some saw as â€Å"free love.† One such idea was the experiment for the superior race through a monitored procedure known as stirpicu lture. Based upon social Darwinism, the eugenics experiment known as stirpiculture caused unrest in and out of the community.   Ã‚  Ã‚  Ã‚  Ã‚  The stirpiculture experiment, named by John Noyes, began in 1869 as a project to create a race of geniuses. Noyes ideology stemmed from Darwin’s Origin of Species which promoted the â€Å"survival of the fittest† (Carden 61). The selection process was vigorous, including submitting an application to a cabinet of central members who would make the final decision of whether the couple would suffice for the experiment (Whitworth 130). A majority of couples selected their own mates, while a quarter were suggested pairs by the committee (Carden 62). The Oneida founder strived to reach this superior race through the careful selection of healthy, beautiful, and intelligent couples. Noyes and the cabinet’s criterion involved being very spiritually refined, while his son Theodore looked more at the physical condition of the prospective candidates. As early as 1859, women were prescribed to enjoy fresh air, the outdoors and the continual development of mental and spiritual qualities (Kern 263). Women were a necessary part of the eugenics experiment, but Noyes and others thought the choice of the fathers was the key to selective breeding (Kern 232).   Ã‚  Ã‚  Ã‚  Ã‚  The women’s ages ranged from twenty-three to forty-two, the men from twenty five thru sixty; often the fathers were ten or more years older than the female participants (Kern 250). One such woman was the niece and lover of John Noyes, Tirzah Miller, she was the embodiment of the ideal woman of the Oneida community, strong in her convictions and firm in the beliefs of the Perfectionist community (Fogarty 17).

First, the car must be completely dry and moved to a cool shaded spot to start the waxing process. Second, take the buffing applicator and add some liquid wax. Now take the applicator and buff the car in a circler motion making sure that no spot is missed on the car. After this is done the wax needs to dry to a haze, which can take anywhere from 20 minutes to and hour. Once the wax has dried to a haze the wax must now be buffed off with some force. The car should now shine and be almost as smooth as glass.   Ã‚  Ã‚  Ã‚  Ã‚  There are a few problems that can occur when waxing a car. Since, the wax has to dry on the car one major problem that one might encounter is the wax drying in the grooves of the car. I waxed my car about three months ago and still have white spots of wax in a few of the grooves. Another problem happens when wax gets on the plastic parts of the cars exterior. When this happens those plastic parts have a look like they have faded making the car look has though it is older and not well-taken car of. Online Classes versus Traditional Classes Essay

Technology and education has given students many choices in how they obtain their education such as online classes. There have been numerous discussions recently about the success of online learning. The success of the online classes versus traditional classes is based upon the uniqueness of each individual way of learning. Online classes and traditional classes provide flexibility, direct communication and course materials. Studies have shown that online classes can be successful and that on average most students perform better than being in a traditional classroom. The argument of online classes and traditional classes; which is better than the other has many exceptional differences. One is the discussion of how does a person learn: is he a visual, auditory or kinesthetic learner? A visual leaner needs written materials, exercises and graphics. An auditory learner has to hear it, repeat it and needs to have a discussion. A kinesthetic learner needs real life simulations, hands on demonstration and needs to see how the principle works. Visual learners and kinesthetic learners most likely would be more successful in online courses because of the atmosphere it provides. Auditory learners would need to have traditional classes which would provide the best learning opportunity for their learning style. There are cases that individuals do not know how they learn best; an assessment should be given in the situation. The second discussion is flexibility. Online classes and traditional classes both can be just as flexible as the other. Online classes may seem more flexible but depending on the class the deadlines of each assignment may be very time consuming. Online classes sometimes provide more work than traditional classes. Traditional classes can be flexible due to the times that you select to take the classes. Schools are providing more night classes and early morning classes for the working and family-oriented students. Traditional classes provide more lecturing and sometimes more hands on in order to teach the concepts. The third discussion is communication. Online classes communicate by sending  and receiving emails. Some classes have chat rooms for discussions which provide the students opportunities to enrich their understanding of the course or subject being discussed. It also allows those individuals that normally would not speak in discussions to express their thoughts. The instant transfer of material is convenient for the students and professors to communicate. A traditional class allows students to verbally exchange ideas and ask questions directly to the professors. Sometimes this environment does not allow the students to fully express themselves due to time constraints. Some people feel that there are communication barriers in a face to face classroom setting due to time and class structure. (Anderson) One of the most important debates between the two classes is the course materials. The question has been asked if the online course is giving adequate material to teach an individual a lesson. Studies show that online professors give more than enough resources to express the subject and also gives practice graded material to ensure that the student understands. Traditional classes have been accused of doing the opposite and taking for granted that the students are in their presence and extra resources are sometimes a rarity. Online classes allow a person to minimize their distractions and it gives them the ability to fully concentrate on the material they are presented with. A person that enrolls in an online course should be self-motivated in order to be in an environment that requires the individual to monitor themselves and be responsible to handle the course as if it was a traditional course. (Kassop) Online courses provide the ability to be accessible at all times but the work that is required should be quality work. The students’ writing ability will enhance because this is their only form of communication. (Kassop) The professors do not expect students to procrastinate because of the accessibility but the possibility is there. The professors of these courses expect more from the online students since there are not time restraints. Online students have the ability to form small groups to relate to because the workload in online course is difficult. The workload is believed to be difficult be cause of the apparent reason of teaching themselves. Traditional classes allow students social interaction which is a vital piece for many students. There is a set amount of time in a classroom setting and there is limited one on one interaction with the professor. Professors have the ability to read their students body language to assure them if the material or concept is being understood by his or her students. Traditional classes also allow students to get behind if they miss a day or an assignment. The traditional classroom allows the students to strengthen various areas of their social skills such as verbally expressing themselves in classroom discussions. There are differences and similarities in online courses and traditional courses. Throughout are reading and research there can be two conclusions: 1. Online courses are better than traditional courses if the individual is discipline to take the course. 2. Online courses and traditional courses are just as good as the other because they both have the same goals and are achieving the goals to the students that are willing to receive. Wii Case

Problem Statement: Sales of the Nintendo Wii have been much higher than initially forecast. As the 2007 holiday season approaches, Nintendo does not have enough capacity to meet demand. The shortage will negatively impact profit and potentially cause lost market share. The immediate issue is adjusting the marketing strategy to handle the product shortage and minimize the negative market effect. Situation Analysis: BACKGROUND: Nintendo was initially founded in 1889 as a card company. The Company moved into the electronic entertainment industry in late 1970. Nintendo went on to release its own home console in the 1980’s which was an instant success, subsequently dominating the video games market in home video games consoles and handheld consoles. In recent years, Nintendo has met fierce competition from Sony and Microsoft. This competition resulted in Nintendo’s market share dropping from 80% in 1992 to a low of 16% in 2003. Nintendo realized that markets, competition, and product life cycles were changing at a fast pace, and most of the profits were going to the innovators. In order to win back market share, Nintendo has developed an innovative new game console named the Wii. OBJECTIVE: The current objective for Nintendo is to effectively develop a marketing strategy to manage product sales through the 2007 holiday season. The long term objective for Nintendo is to maintain its market leader status, communicate customer’s needs in an effective way and fulfill customer’s expectations. S. W. O. T. ANALYSIS: Strengths: Offer differentiated products from its competitors; †¢In 2006, Nintendo won more awards than other company, including â€Å"Best of Show† award, and â€Å"Best Hardware† award; †¢More competitively priced ($249) than its competitors while still earning a profit ($50) as the Wii console is inexpensive to produce; †¢Brand name recognition (short, simple, easy to read and remember, internationally understood, and suggestive of product benefits—â€Å"ii† images gathering people to play); à ¢â‚¬ ¢Strong support from game evelopers to shift more resources to the development of Wii title; †¢Constant media coverage from newspaper headlines drew attention to the Wii console; Weaknesses: †¢Nintendo lost significant market share after it reached its peak of success; †¢Inability to keep up supplies with international market demand; †¢Does not provide extreme high definition and DVD/Blue-ray capability; †¢Negative media-individuals had injuries due to the controllers slipping out of users hands; Opportunities: A recent movement in society towards healthier lifestyles. Wii meets the demand by incorporating physical movements when playing the game; †¢There are only two competitors which directly compete against each other with graphics, leaving room for Nintendo uses a different selling feature; †¢Potential market for video game consoles for non-traditional age groups (casual gamers and non-gamers) over age of 35; Threats: Consumers might turn to competitor’s product to satisfy their wants due to the Wii shortage problem; †¢Sony and Microsoft may offer more attractive deals to fight back for market shares; †¢Substitution is a significant threat and can occur within the existing competitors. Conclusion Nintendo offers an innovative product that offers good value to consumers. The threats from competitive brands are high. However, Nintendo is offering a unique product which is in high consumer demand.
